Yes, I am in-network with Premera, Anthem, and most Lifewise and BCBS plans. Otherwise, I can provide a “superbill” receipt for you to use with your insurance provider for potential reimbursement.
-
This depends on your insurance provider and your specific plan. I recommend you reach out directly to your provider and ask if they reimburse for out-of-network therapists, at what rate, and what you need to provide to access reimbursement. Some questions to consider asking your insurance company:
Are out-of-network providers accepted for mental health services?
What is my co-insurance rate for out-of-network providers?
What is the UCR (usual and customary rate) for CPT code 90837 in [your zip code]?
How much do I pay towards my deductible before my benefits will apply?
How do I seek reimbursement for out-of-network services?
